• This is where our established patients can access certain documents from us upon request. Please keep in mind that it must be activated by our staff in order for you to access it. This is normally used when our patients need their paperwork transferred to another eye doctor because they have moved or if another specialist needs their history.

• Please allow up to a week for our doctors and staff to get your documents ready when you request them.

Password: Is normally your first initial, last initial and full date of birth without spacing. (i.e. tj01012015)
